Step 2 - Enclose all the necessary proofs and supporting documentation

Obtain and enclose all necessary proofs

You will be required to enclose several items of proof with your completed application form:

1. Certified copies of your Qualification(s)

2. Completed and signed statutory declaration

3. Certificate of Criminal Clearance (only applicable if you have lived abroad for 6 months or more)

4. Completed and signed Garda Vetting Form

5. Certified Copy of your Passport

6. Certified copy of your birth certificate, adoption certificate or other evidence of place of birth (as applicable)

7. Certified evidence of any change of name (if applicable)

8. Two certified Passport sized photographs

9. Annual Accounts and Notice of Assessment from the Revenue Commissioners (only applicable if you are self employed to show you have been engaged in practice).  

10. Proof of Professional Indemnity Insurance  (only applicable if you are self employed to show you have been engaged in practice).  

11. Additional information in support of your application (if applicable).

Note: Certified copies means certified by a Solicitor as a true copy of the original. In the case of a qualification certificate the principal of the educational institute that granted the qualification must certify the document as a true copy of the original.
